You can use your Annual Wellness Visit to gain more focus on your overall well-being
and your physical, mental and emotional health.

What to Expect

During your Annual Wellness Visit, you and your doctor typically discuss a plan for preventive care in the coming year. Your doctor may also:

1. Conduct a general health exam of your heart rate, blood pressure, respiration, etc.

2. Give you a health-risk assessment questionnaire about your personal stats, medical history, lifestyle, etc.

3. Take your medical and family history.

4. Discuss your current medications.

5. Discuss any risk factors you should be aware of.

6. Screen for any cognitive impairments.

7. Create a immunization and screening schedule
for the next 5-10 years.
Please take the following information with you to your appointment.
Any medical records you may have, including screenings and shots you’ve had in the past year.
Your family health history.
A list of all prescriptions, over-the-counter medicines, vitamins and supplements you take.
A list of all your doctors.
Please bring your photo ID and insurance card to each appointment.
