We are currently accepting new patients and would like to welcome you into the Petoskey Family Medicine Family. Carolyn Wiese, PA-C and Nicole Wolf, PA-C are currently accepting new patients of all ages. Dr. Scott Hotchkiss and Dr. Samuel Minor are unable to take new patients at this time. To become a patient, please fill out the new patient packet and mail or return to our office. If you would like a packet mailed to you, please call our office. Once the packet is returned, your chosen provider will review your information and you will be contacted to schedule an appointment.
