Missed Appointment Policy


Unless there are extenuating circumstances, a 24 hour notice of cancellation is required for all scheduled appointments.

Any patient, who fails to arrive for a scheduled appointment without cancelling the appointment prior to 24 hours notice, will be considered a "no-show". A patient, who is a no-show more than three times, may result in being dismissed from the practice. A no-show may also result in a $50 fee.

A patient who arrives more than 10 minutes after their scheduled appointment time, may have to reschedule. A call to verify availability is appreciated.

Financial Policy

1. Patients need to present their insurance card(s) and a photo ID at each visit.

2. Payment is due on the date of service for any copays, co-insurance, and deductibles. Payment is accepted by cash, personal check, and credit & debit cards. HSA and health reimbursement account cards are also accepted.

3. Petoskey Family Medicine, PC does participate with the following insurance companies: Medicare and Medicare Advantage Plans, Blue Cross and Blue Shield, Blue Care Network, Priority Health, Aetna, Cigna, McLaren Health, United Health Care and Humana.

4. A courtesy claim will be billed for all insurance companies; however, the patient may owe a larger amount if the insurance company uses a preferred provider network and the patient is not seeing an in-network provider.
